Adult Day Health - a traditional care model providing Adult Day Health to individuals that wish to age in place at home and in their communities. The care coordination assists with the management of chronic disease and improving health outcomes. Memory Care/Respite - A community based day program providing a dementia specific model of service that improves care for individuals with a diagnosis of Alzheimer's disease or other neurocognitive disorder.

  • Adults living in McHenry County and Northern Kane County. Eligibility requirements: Adults age 18 and over for Behavioral Health, Adults age 50 and over for Adult Day Health and Memory Care day programs.

  • Hours are 9:00AM -5:00 PM Monday through Friday.
  • Spanish interpreter available upon request.
  • Services funded through Medicare, Medicaid, Insurance, MCO's, VA, IDOA or private pay, sliding scale available based on income eligibility.
