If you would like to submit a FOIA request, please carefully read the information below. Illinois law requires public bodies to comply with or deny a FOIA request within five business days, unless the time for response is extended for an additional five business days, as allowed under the law.   A request for records to be used for a commercial purpose requires a public body to respond within 21 business days.

Please be specific in the nature of your request in order for FOIA Officers to process your request more efficiently. (Illinois law does not require you to submit on the FOIA Request Form, but the request should be in writing).  *Electronic records can be requested and will be provided, as allowed under the law

Copy Fees (Some fees are regulated by state statutes):

· First 50 pages are free (black & white, letter or legal size)

· 15 cents per page thereafter (black & white, letter or legal size)

· Actual costs of copying for color copies or other storage media

RECORDS AVAILABLE UPON IMMEDIATE REQUEST:

All board meeting minutes approved by the McHenry County Mental Health Board are available on our website.  Annual reports and three year plans containing statistical data and summary financial information are also available on our website. Requests for other materials should be made in writing to the lead FOIA officer.

 

RECORDS ON FILE:

· Contracts approved by the Mental Health Board

· Funding applications are available after funding decisions have been finalized by the mental health board.

· Periodic financial reports approved by the Mental Health Board.

· Responses to specific request for proposals (RFP/RFQ) are available after vendors decisions have been finalized by the MCMHB

· Annual audits of the MHB are presented within the County’s Consolidated Annual Financial Report available through the county.

· Funded agencies lists and phones numbers, contact information (available on the web site).

· Board approved organizational chart

· Board approved policies

· List of board members and contact information

 

VENDORS:

List of vendors used by the Mental Health Board are part of the County’s Vendor list available through the county.

FUNCTION:

The Illinois Community Mental Health Act mandates that each Community Mental Health Board in the state:

· Review and evaluate community mental health services and facilities

· Plan for programs of community mental health services and facilities

· Consult with others regarding the most efficient delivery of services

· Appropriate funds to maintain mental health services and facilities

 

The Board realizes that it exists at the will of the community as expressed by passing the referendum. The Board is mandated to assure on behalf of the community that those in need of mental health, developmental disability or substance abuse services will have those services available to them. To accomplish this task the Board must continuously engage in policy development focused on their governance of all access, quality and cost concerns for their constituency's behavioral healthcare needs.
