Question: In preparing the application for accreditation we are still CARF pending, it that what you want entered for number 8? Answer: Yes Question: The therapists are both licensed and EI credentialed, should the credentialing information be entered for individuals in number 10 under certification? Answer: No. Item number 10 relates to the certification of the agency not the clinicians. As an example, if the agency is Medicaid Rule 132 certified, then Medicaid would be listed along with the date of expiration of that certification. Staff credentials are entered in the personnel listings. |
