Agency Funding Application FY ‘11

 

Welcome to our McHenry County Mental Health Board Agency Funding Webpage.  On this page you will find the necessary documentation and forms to complete an application for the upcoming 2011 Fiscal Year.  Our funding is open to three tiers of funding.  Each of these tiers are detailed in the Tiers of Funding document.  As a reference please review our “About Us” on for copies of our Three Year Plan (2009-2010-2011) and 2011 Annua Goals and Fudning Guidelines.  We thank you for your interest and desire to serve our residents of McHenry County.

 

Tiers of Funding:  Fiscal 2011 McHenry County Mental Health Board Funding Tier Guidelines:

 

Tier 1: Incumbent Provider Contracts for Core Services

Requests for Funding that represent Developmental Disabilities Services; Psychiatric Services; Mental Health Court positions; Inpatient Hospital Psychiatric Services – Centegra; Crisis Services; and TBI Services will be recognized as continuation funding from adjusted FY10 base contract levels (adjusted for anticipated FFS Set Aside Volume as recommended by staff).  The Board may award cost of doing business increases subject to funds availability.  A full application is not required, but Board and staff will review work plan, updated agency information; Board and staff information, Revenue and Expense documents of the application, service levels and outcomes for the year end and for the coming year.  Limited to incumbent contractors.  No new applicants.

 

Tier 2: Independent Contractor Agreements for Supportive and Individual Services

Current agreements will be converted to FY 11 independent contractor agreements in which the MHB approves specific individual client services or in situations where access is required by the Board for a specific linkage.  Funding is limited to current FY10 contract levels and use of set aside funds.  This includes the following current providers.  These require funding through a Warrant of Need Process or other specific client approval process.

             Adult/Child Rehab

             Allendale

             Consumer Credit Counseling

             Horizons

             NISRA                                        

             Family Health Partnership

             Northern Illinois Counseling Associates

             The Mathers Group

             Professional Consultations including referrals to PRA

 

These agencies are required to complete sections of the FY2011 Application.

 

Additional contracts can be considered during FY11 in order to support immediate and urgent access and will be funded under the “Support to Clinical Services” line item and Fee for Service Set Aside funding as appropriate.  New contracts will be approved by the Executive Director for amounts under $30,000 and to the Board for amounts over $30,000. 

 

Tier 3 – FY11 Open Funding Application

Eligible organizations that have been pre-approved by the Mental Health Board based on the Funding Guidelines contained herein may submit a fully executed FY11 Funding Application due on the following schedule:

New applications from new agencies Friday, August 6, 2010 at 10am.

New programs from currently funded agencies Friday, August 20, 2010 at 10am.

Continuing programs from currently funded agencies Friday, August 20, 2010 at 10am.

 

The funding applications must align with MHB goals and strategies as outlined in the FY 09-10-11 goals and are highly prioritized to the goals identified within the FY11 Goal Statements approved by the MHB on April 19, 2010.  Special consideration is made for applications that demonstrate high collaboration, evidence-based practices and demonstrated outcomes, and cultural competency strengthening the system of care and recovery principles.  Prioritization of funding may be subject to urgency of need; impact on system access; and/or applicability of leverage for other funds for the program.

 

Special Instructions for New Organizational Applicants:

All organizations seeking first-time applications for funding must attend a pre-qualification meeting held on July 12, 2010 at 2:00 p.m. at the Mental Health Board facilities located at 620 Dakota Street.  The organization must bring copies of their Articles of Incorporation, Corporate Status, (i.e., 501c3 exemption); organizational chart; documents of licensure and accreditation status; prior year’s financial audit as exhibits to the application.  Organizations may contact the Mental Health Board for additional information prior to this meeting.
